Personally, I would pay 40 dollars only after the features mentioned above have been included. There are other apps which only costs max 5 dollars and do almost as much as Narrator. What about reading dates and years? Family names? Street, countries, squares, rivers names?

Just try to teach the app how to pronounce "Venice" in English, or to read "Schönberg" in German or "Seine" in French and see what happens.
However, for 40 dollars (20 now for a few more hours) the application really needs some professional-grade features not available in the current version. It's too bad, because the application in itself is a very good start. I don't expect Mariner to change their attitude in the future. Mariner could have improved this application in many small and larger ways but never did.
